This presentation highlights the importance of an integrated, organization-wide approach to reentry in achieving improved outcomes for individuals returning to the community. Reentry efforts are frequently challenged by limited funding, staffing shortages, and constrained resources, resulting in fragmented or “one-size-fits-all” practices that fail to meet individual needs. Such approaches reduce the effectiveness of reentry services and increase the risk of recidivism and return to incarceration. By incorporating early planning, organizational self-assessment, and coordinated service delivery, an integrated reentry model can help mitigate these challenges. This session will examine how thoughtful planning, individualized support, and strong community partnerships can strengthen reentry practices and support successful reintegration.
