Jacqueline Cruzado, a public health professional with over 30 years direct service, supervisory, and management experience in the field of correctional public health, health education, and disease prevention, curricula development, program management, and team building across systems. As owner of ACOJA Consulting, she provides consulting services and addressed community collaborations in NYC, Camden, Paterson, Raleigh, Las Vegas, and across the islands of Puerto Rico. She is skilled in program design and development, translational research and jail-based population management. Bilingual in Spanish and English, she built, implemented, adapted and sustain collaboratives across systems including primary care, housing, employment, mental health and substance use treatment providers, law enforcement, correctional facilities, community corrections, and more. She served NYC Correctional Health Services as Director of Reentry & Continuity Services, managing federal grant funded programs and was instrumental to the development expansion and enhancement of Transitional Care Coordination, a nationally recognized evidence-informed intervention.
