This roundtable will discuss a holistic approach that expanded access to care in the community for formerly incarcerated individuals and reduced the number of justice responses to issues such as substance use disorders and mental health challenges. The program demonstrated a significant decrease in recidivism in the Indiana Department of Correction by emphasizing care continuity related to social determinants of health.
