Achieving diagnostic fidelity is of paramount importance in providing structured care for patients. Updating the master problem list and reconciling medications are foundational steps for ensuring safe, effective, and efficient health care. Although it is important to add appropriate diagnoses and medications for patients, it is probably more important to "de-diagnose" obvious errors and to deprescribe medications. Specific sentinel diagnoses and medications will be addressed.
