Description

There are multiple unique challenges for facilitating end-of life care discussions in the correctional setting. Learn about a comprehensive program model that uses a partnership of providers and incarcerated people to promote advance care planning. Such a program can build trust and community, create a network of support, improve patient and family experience, decrease staff burnout, improve clinical safety, and reduce unwanted and unnecessary ED and hospital utilization. The presentation reviews program structure options, participant selection, training, implementation, feedback, and subsequent enhancements.
