Ms. Laketta Hubert is the regional director of the Transitional Healthcare Team with Centurion of Indiana. She has over 10 years of experience in the mental health field, specifically working with those who have a serious mental illness and require 24-hour supervision and assistance, those with serious mental illness who are also experiencing homelessness, and those who are experiencing a mental health crisis and in need of short-term stabilization. Ms. Hubert's passion for advocating for those who are often overlooked and underrepresented has led her to this work with the incarcerated population. Her desire is to grow the Transitional Healthcare Team in order to offer prerelease coordination for everyone released from Indiana correctional facilities.
