Successful reentry must include linking individuals with community resources, including health care. Since 2017, a collaborative between the Louisiana Department of Health and the Department of Public Safety & Corrections resulted in over 15,000 people from 18 facilities applying for prerelease Medicaid enrollment. This presentation will detail how the program works and review CMS guidelines for Medicaid during incarceration, comprehensive program logistics, staffing roles, system-level requirements, fidelity monitoring, and lessons learned.
