This presentation describes a protocol for transitioning to buprenorphine for maintenance treatment of a patient's problem opioid use. Long-acting formulations of buprenorphine may provide advantages over sublingual buprenorphine products. One advantage is that improved compliance should lead to more consistent blood levels and therefore greater effectiveness in preventing relapse. Long-acting formulations also should provide significant protection against misuse and diversion.
