With the advancement of technology, jails in Colorado have partnered with a statewide health information exchange organization to develop databases of information within EMR systems to achieve continuity of care goals. A jail-to-community transition program was designed in an advanced technology system to network with the HIE to begin data access and integration from time of booking through discharge. Jails with very limited resources were able to increase access and improve care levels.
