RT-06 An RN-Led Primary Care Program at an Urban Correctional Transition Center
Description
This roundtable will discuss an initiative to establish an RN-led clinic at a correctional transition center. The clinic is a collaborative effort between a College of Nursing, correctional transition center, and a community health center. The discussion covers establishing relationships, project development, contract process, phased-in approach (assessments, education, care coordination, telehealth), and results. Empowering residents to manage their own health and establish care with community-based providers is a critical part of successful reentry.
