This case study describes navigating obstacles to form a therapeutic patient relationship with a patient with a personality disorder, hypertension, poorly controlled diabetes, and chronic foot ulcers. The medical team overcame treatment refusals, transfer gaps, and other barriers and discovered and treated a very rare infection.
