Medical charting is certainly a chore, and time constraints often contribute to inadequate charting. But the health record is often the key evidence in a lawsuit concerning patient care. This talk will examine how simple changes in charting practices can turn a difficult case into a winnable one, problems that arise due to unfortunate word choices in depositions, and how being too talkative can create liability traps.
