454b Improving Care for High Utilizers of Urgent and Hospital Care*
Description

High utilizers of the urgent care clinic with repetitive visits and transfers to the emergency room/hospital are common in correctional settings. By making a personalized plan of care, we have been able to involve medical providers, nursing, and custody staff to align with the goal of taking care of the patients' needs in housing or in primary care clinic. This has resulted in a standardized approach to patient care and behavioral needs.

Educational Objectives

  • Recognize the need to identify patients with frequent acute care visits with repetitive chief complaints
  • Summarize a document containing the patient’s medical history, most common chief complaints, and how they have been addressed
  • Design a plan of care for all providers in the correctional health system to follow, regardless of clinic
