Description
This talk will describe the human-centered design and pilot testing of a reentry care transition program: Granite State Reentry Support for Empowered Transitions in New Hampshire. Topics include creative strategies to reduce silos between carceral facilities, community health care, and social services and enhance cross-system collaboration during reentry. The Granite State RESET program's strategic partnership across five organizations will be used as a case example.
