This seminar will examine practical, evidence-based strategies for sustaining engagement in HIV care during and after transitions from custody to the community. The presenter will explore approaches to strengthening linkage and retention, supporting medication adherence, addressing stigma and other social and structural barriers, and coordinating care across correctional and community settings. Through case-based discussion and real-world examples, participants will identify opportunities to implement patient-centered strategies that promote continuity of care, sustained viral suppression, and improved long-term outcomes for people with HIV. 1.5 CE credits
