Dr. Jacob Sermeno is a licensed clinical social worker with over 10 years' experience working with individuals with chronic mental health concerns, cognitive deficits, and aging challenges. While pursuing his bachelor's and master's in social work, Dr. Sermeno focused on the aging population and worked in senior care, hospice, and nursing home social services. After recruitment to Canyon Ridge Hospital to develop and sustain their geriatric psychiatric program, Dr. Sermeno gained experience working with clients of all ages experiencing chronic mental health concerns. Currently working as a clinical supervisor at San Bernardino County Jail, Dr. Sermeno has been developing the jail's mental health treatment and discharge planning program to provide individuals with serious mental illnesses greater access to resources when returning to the community. He strives to assist clients on their path towards higher functioning and fulfillment of their goals.
