135 Improving Hypertension Through Education and Lifestyle Modification
Description

Nearly half of all adults in the United States have hypertension. Untreated or uncontrolled, it is the single largest contributor to cardiovascular disease, causing stroke, heart failure, coronary artery disease, and kidney disease. In addition to poor physical health, many justice-involved individuals have limited to no health literacy. This session describes a multidisciplinary effort to educate incarcerated patients on hypertension and improve health literacy, lifestyle decisions, and adherence to care plans, in addition to reducing sick calls and increasing attendance at chronic care appointments.

  • Design a successful action plan to address patient health issues such as hypertension
  • Describe the resources needed to address patient health issues
  • Identify stakeholders who can help promote the action plan
