Many adolescents have multiple admissions to juvenile detention centers. The admissions are often a result of trauma, safety concerns, poor health or education equity, or insufficient social determinants of health. Connecting youth to care management may improve the areas that often result in incarceration and therefore minimize future admissions to juvenile detention.
