Description
This presentation describes successful efforts toward improving health strategies for patients with chronic kidney disease in West Texas prisons. Multiple clinical factors were addressed to enhance a coordinated care approach to CKD management, including reference lab transitions, electronic medical record modifications, provider and patient education, and a protocol for drug therapy management by clinical pharmacists. This project can serve as a template for other primary care clinicians.
