This session explores San Luis Obispo County Jail's experience implementing CalAIM's CMS 1115 justice-involved waiver, which allows Medicaid coverage for eligible individuals 90 days prior to release. Presenters will share strategies for connecting releasing individuals to physical, behavioral, and social health services, billing Medicaid, coordinating with community providers, and meeting regulatory requirements. Attendees will gain practical insights on infrastructure, staffing, data sharing, and transforming correctional facilities into patient-centered health homes supporting successful community transitions.
