Description

This discussion will highlight the ways multidisciplinary collaboration between custody, behavioral health, and restorative justice addresses the complex needs of juveniles in the justice system and contributes to a more holistic and effective approach to rehabilitation. A multidisciplinary team will discuss the challenges and benefits of developing their team approach and implications for continuity of care and successful youth reentry.

  • Provide an overview of the Horizon Model of Treatment
  • Outline challenges encountered and strategies to overcome barriers
  • Introduce a model of transitional housing or restorative residences to promote continuity of care upon reentry to the community
