Description
The transition from incarceration to the community presents significant challenges for individuals with mental health conditions and HIV, often leading to disruptions in care, increased health risks, and adverse public health outcomes. This session will explore the critical gaps in post-release mental health and HIV care, emphasizing the impact of fragmented services, social determinants of health, and systemic barriers to continuity of care.

- Discuss successful models and best practices for improving post-release linkage to mental health and HIV care
