Description

As the number of incarcerated patients with chronic conditions increases, providing quality care becomes increasingly challenging due to time and resource constraints. Integrating a clinical pharmacist into the chronic care team at Salt Lake County Jail over nearly five years through virtual consults has improved efficiency and enhanced patient care. We will review the strategies employed and the observed clinical improvements. Attendees will learn about the benefits that clinical pharmacy teams can bring to correctional health care.
