458b It’s Essential: Continuity of Care after Incarceration
Description

To maintain the benefits of care provided while incarcerated to the public and the patient, continuity of care needs to be addressed through transitional care coordination and other evidence-informed interventions.  The speakers will explore the distinct challenges in facilitating care coordination across systems and offer tools and tips, model approaches, free resources, and funding opportunities while highlighting the benefits of jurisdiction-wide collaborations.

  • Identify steps for initiating, developing or enhancing services that support continuity of care before, during and after incarceration
  • Describe barriers and facilitators to continuity of care
  • Discuss specific evidence-based approaches that address key social determinants of health as part of successful community reintegration
