429 Geriatric Care for Chronic Conditions*
Description

Chronic conditions are on the rise in both general and incarcerated populations, but incarcerated older adults have disproportionately higher rates of early onset, comorbidities, and accelerated aging. Startling discrepancies in rates of cognitive decline between general and incarcerated populations prompt a need for adapted models of geriatric care. This talk will discuss implementation of multifaceted models of support for geriatric patients suffering from chronic conditions.
