Advance care planning is a best practice that includes health care proxies and POLST forms. Based on a review of 10 years of medical records for a large regional hospital, the presenter found ACP for incarcerated patients was significantly lower than for the general population. In this presentation, she discusses the study's findings, reviews an ethics consultation case example, and addresses barriers to ACP.
