This session explores the importance of cross-sector collaboration to support individuals transitioning from jail back into the community. It addresses challenges faced by people with complex health, behavioral, and social needs, including substance use, mental illness, and homelessness. Attendees will learn strategies for improving care coordination, creating warm handoffs, and fostering collaboration between teams to enhance quality of life, reduce recidivism, and promote safer, more supportive reentry outcomes.
