This talk will describe the human-centered design and pilot testing of a reentry care transition program: Granite State Reentry Support for Empowered Transitions in New Hampshire. Topics include creative strategies to reduce silos between carceral facilities, community health care, and social services and enhance crosssystem collaboration during reentry. The Granite State RESET program’s strategic partnership across five organizations will be used as a case example.
