Ms. Sage Elle Hapke is a program manager of trans services at the San Francisco Community Health Center, where she oversees programming that connects transgender and gender nonconforming individuals to health and wellness services, including behavioral health, primary care, gender-affirming care, HIV prevention and treatment, substance use treatment, permanent supportive housing, and reentry support. She is a woman of trans experience and has supported transgender community members in San Francisco for more than a decade. Ms. Hapke serves as board treasurer for the Castro Country Club, a sober community center. She previously helped open the nation’s first transgender homeless shelter, the Taimon Booton Navigation Center, and taught classes at San Quentin State Prison through the Prison University Project. She is a Master of Social Work candidate at California State University, East Bay, and holds a Bachelor of Arts in critical diversity studies from the University of San Francisco.
